Important Information for Medicaid Patients

Thank you for considering Aasar Family Dentistry. We’re happy to see Medicaid patients, but before we can schedule your visit, please review the following important details to ensure your plan is eligible.

We Currently Accept the Following Indiana Medicaid Dental Plans:

If your plan is not one of the four listed above, we are not in network and unfortunately cannot see you under Medicaid.

Please Make Sure You Have a Dental Plan

Many patients send us their medical insurance card, but this is not the same as dental coverage. We need your dental plan information to verify eligibility and schedule your appointment.

What You Need to Do:

  1. Confirm that you have active dental coverage under one of the accepted plans above.
  2. If you’re unsure, call Indiana Medicaid and ask:
    “Do I have active dental coverage through DentaQuest/Anthem, CareSource, MDwise/Delta Dental, or Envolve/Centene?”

If you don’t currently have dental coverage, you may be able to activate it — but this must be done before your appointment.

When You Call or Text Us, Please Include:

We verify all coverage before your visit. If your plan is inactive or not eligible, your appointment may be canceled.

No Dental Coverage? We Can Still Help.

We offer affordable self-pay rates and a membership discount plan for patients without insurance.
